Make a Referral - Out of the Blue

COVID19 UPDATE: We have a new online creative offer called HOOT from home that you can access from a laptop, smartphone or tablet. We're also developing new ways that people can take part in Out of the Blue without internet access.
Once we’ve received your referral, we’ll ring you within 3 working days to have a chat about what you’d like try. Alternatively, you can complete your referral over the phone with one of our CSWs by calling the following numbers:

1 - 3 PM Monday & Wednesday: Jonathan (07833 232089)
1 - 3PM Tuesday & Thursday: Kathy (07880 731767)

If you are forwarded to voicemail, please text the number to let us know you would like to be called about your query.

We hope to see you soon.

If someone you support is interested in attending, please fill in the referral form below. You can be referred by your GP, keyworker, CPN or support worker or individuals can refer themselves.

Personal information will be sent securely to the team at HOOT who will be in touch to make sure we have everything we need and to invite ther person you have referred to sessions.

If you’re not sure which sessions you might like to attend, you can check out the ‘What’s On’ page to see what is happening in the groups or call us on 01484 516224 to chat with a member of staff about what’s currently on offer.
